*Due to a shortage in testing supplies, we are only performing COVID-19 tests for high risk/symptomatic patients at this time.  We apologize for the inconvenience. 

The Brazoria County Employee Health and Wellness Clinic operated by Next Level Health and Wellness. Appointments are available and can be booked online.  Appointments are encouraged, but if you walk in, we will see you as quickly as possible.  Clinic hours are 8 am-4 pm Monday-Friday.

Labs and Nurse visits: 7-8 a.m.
Clinic visits with provider: 8 a.m. – 4 p.m. M-F
We do not close for lunch.

 

Outside Lab Orders- please fax or drop off your lab slip at the clinic and allow 48 hours to schedule your lab draw.

Meet Our Provider: Ofon Ntia, APRN, FNP-C

Ofonime Ntia, APRN, FNP-C was born in Nigeria and moved to the United States when he was 20 years old. Ofonime attended Oklahoma State University, in Oklahoma City, Oklahoma, where he received an Associate Degree in Nursing, and then moved on to receiving a Bachelor of Science Degree in Nursing, at The University of Central Oklahoma. While working in the Liver Transplant and intermediate care unit at Integris Medical Center, Oklahoma City, Oklahoma, he completed his Master of Science in Nursing with a subspecialty in Family/Primary Care at Maryville University, St Louis, Missouri. Ofonime is a Board Certified Family Nurse Practitioner who is dual licensed in both Oklahoma and Texas. Ofonime has over 12 years nursing experience and his experiences as a bedside nurse working in long-term care, skilled nursing, and on the Liver transplant floor, have all shaped his practice to work with families and teach them about maximizing their health and well-being both physically and cognitively. When Ofonime is not caring for patients you can find him reading, playing basketball, driving sports cars, being a serial entrepreneur, spending quality time with his family, and traveling around the world.Ofonime and his wife, Queena, with their beautiful daughters and son reside in Houston, Texas.

Meet Our Provider: Haala Hai, DO

Haala Hai, DO was born in Hyderabad, India but grew up in Flint, Michigan. Haala obtained her Bachelor of Science degree from the University of Michigan, and her Doctorate from UNT-TCOM. Haala completed her residency in Houston, TX at Memorial Family Medicine Residency Program, specializing in Family Medicine and has 5 years experience in the medical field. A few of Haala’s most notable achievements include participating in community involvement, quality improvement, breast cancer research, and clinic flow and time research. 

When Haala is not caring for patients you can find her organizing spaces, Hiking, scrap-booking, or trying new recipes. Haala currently resides in Houston, TX with her husband and two kids, Omar (6) and Nura (2) .
