CEO and Medical Director
Juliet Breeze, MD graduated from Northwestern University in 1991 with a Bachelor of Arts degree with honors in English. In 1995, she earned her medical degree from Baylor College of Medicine. She completed residency training in Family Practice at Memorial Hermann Southwest in 1998 where she received the Norma Culver Outstanding Family Practice Resident award. She founded a private practice in Richmond, Texas and worked in group practice for four and half years before leaving to pursue the business aspect of medicine.
She served as the CEO of Richmond Bone and Joint Clinic for ten years and the President of Kingsland Surgery Center for three years. She has project-managed all aspects of design, construction, and operations for several hospitals, surgical centers, and physician practice start-ups.
Currently she serves as CEO and Medical Director of Next Level Urgent Care, LLC where she is able to combine her business skills with her primary care background. In Next Level Urgent Care she has combined her customer experience focus with her dedication to quality and consistency of care.
Dr. Breeze enjoys traveling, language study, and cooking in her free time. She also enjoys time with her husband, children and host of pets.
Terence Chang, MD grew up in west Houston, attended the Wharton School at the University of Pennsylvania, and received his medical degree at UTMB in Galveston. He completed his residency at Memorial Family Medicine Residency Program where he served as one of the Chief Residents. He then did his primary care sports medicine fellowship at Bayfront Medical Center where he served as team physician for the University of South Florida in Tampa and Eckerd College in St. Petersburg. He then returned to Memorial Family Medicine Residency Program as faculty, taught medical students from Texas A&M and UTMB in his role as clinical assistant professor, and practiced at Physicians at Sugar Creek and Memorial Hermann Southwest Hospital.
He continues to be a team physician for Houston Baptist University (NCAA Division 1). He leads the finish line medical team for the Houston Marathon and led the team during the 2012 U.S. Marathon Olympic Trials in Houston. He also provides medical coverage for Ironman Triathlons and consults for local area sports teams. In addition to general medicine, Dr. Chang takes interest in diagnosing, managing, and preventing athletic injuries.
Wil Jeudy, MD completed his medical school at UT Health Science Center - Houston and completed his residency in Family Medicine at Memorial Hermann Southwest Hospital. He has also completed extra training in Medical Bariatrics. He was raised in the Rio Grande Valley, and is fluent in Spanish. His special interests include live music, photography, cooking, and travel/exercise when time allows. He is generally awesome.
Dr. Karen Rakers is originally from rural Illinois, and attended Saint Louis University for undergraduate studies and medical school. She completed her residency at MacNeal Family Medicine in Berwyn, IL and has since practiced general family medicine and been a consultant in compliance review. She is Board Certified in Family Medicine and is a member of the American Academy of Family Physicians.
Dr. Rakers is married with three children and lives in the Sugar Land community. Outside of work, she enjoys spending time with family and friends, exercising, traveling, and exploring the Houston area.
I was born in Houston and raised in Fort Bend County. After high school, I attended Prairie View A&M University where I received a degree in Biology. I then attended The University of Texas Medical Branch where I received my degree in Medicine. I am board certified in Family Medicine. In my free time I enjoy spending time with my family, eating out, and seeing shows in the performing arts.
Robbyn L. Traylor, MD is a 6th generation Texan who was raised in Fort Bend county. She graduated from Kempner High School in 1997, and matriculated to Prairie View A&M University where she graduated with a Bachelor of Science degree in Biology. After this she attended the University of Texas Health Science Center at San Antonio School of Medicine where she graduated in 2005. Dr. Traylor attended the Wesley Family Medicine residency program, and is now a board-certified family physician. During her professional career she has gained experience in primary care, emergency medicine, and immediate care. She is grateful for the opportunity to provide quality care for patients of all ages in the greater Houston area. In her spare time Dr. Traylor enjoys reading, cooking, and spending time with her family.
Margaret Umah, MD is a board certified family physician who attended the University of Missouri for undergraduate and medical school. She then relocated to the Houston area, and graduated from Memorial Family Medicine Residency program. After graduating from residency, she began her practice in Richmond, Texas. She has had the privilege of working in medical missions in Nigeria. She is fluent is medical Spanish. She currently lives in Sugar Land with her husband and 3 sons.
Brandon Wiese, DO is a former Midwesterner who has migrated to Texas. He was born and raised in Iowa and graduated from Creighton University with a Bachelor's degree in Biology. He received his medical degree from the Kansas City University of Medicine and Biosciences.
Dr. Wiese is a recent graduate from the Memorial Family Medicine Residency Program in Sugar Land, TX. He is an avid sports fan and has a background in computer repair. Dr. Wiese lives with his wife, an Obstetrics/Gynecology resident and are expecting their first child in December 2014.
Lisa Attales, ENP is a native of Louisiana where she attended LSUMC in New Orleans and graduated with her BSN in 1991. She then traveled and explored the United States from coast to coast working in numerous Emergency Departments for a few years. She came to Houston in 1999 to pursue her masters degree an UTHSC and graduated from the Emergency Nurse Practitioner program in 2001. She has worked in Emergency Departments throughout the greater Houston area for the past 13 years. In her free time, she enjoys traveling, playing with animals, cooking, and having fun with family and friends.
Rita A. DelloStritto, PhD, RN, ENP was raised in the Fingers Lake Region of New York State, in the Town of Owasco. After graduating from high school, she joined the United States Air Force, where she served four years as a Medical Service Specialist. Her experience in the USAF was the catalyst that lead her to pursue a career in nursing. She received her Bachelor’s Degree in Nursing from Texas Woman’s University, in 1984. She worked as an RN in an Emergency Department in the Texas Medical Center. In 1989 she completed her Master’s Degree of Nursing, as an Emergency Clinical Nurse Specialist, from the University of Texas Health Science Center – Houston. In 1995 she completed a Post-Mater’s certificate as an Emergency Nurse Practitioner. She has worked, primarily in Emergency Care for 30 years. In 2005, she earned her Doctorate in Nursing from Texas Woman’s University. She is currently an Associate Professor at Texas Woman’s University and is the Track Manager for the Acute Care Nurse Practitioner Program. Dr. DelloStritto is certified as an Acute Care Nurse Practitioner. She enjoys photography, travel, and spending time with her family and friends.
Karen DuBose, ENP grew up in a military family, spending much of her youth in Japan.
Early on, while raising her children in San Antonio, she worked as a jeweler and goldsmith.
She received her BSN from the University of Texas Health Science center in San Antonio and later moved to Houston where she earned her MSN in the Emergency Nurse Practitioner program from the University of Texas Health Science Center. She has been involved in healthcare for over 20 years.
Karen enjoys traveling, reading and spending time with friends and family.
William Johnson, FNP was born and raised in Texas. He earned his Bachelor’s Degree from University of Houston. He graduated from the University of Texas Medical Branch at Galveston with his Master’s Degree in Nursing and is board certified as a Family Nurse Practitioner. William is a member of Sigma Theta Tau Honor Society, Texas Nurse Practitioners, and the American Academy of Nurse Practitioners. William enjoys cycling, hunting, and spending time with his family.
Christopher Jones, NP graduated from Bradley University in 1994 with his Bachelor's of Science in Nursing. Upon graduation he began service in the Army. He was honorably discharged in 2003, serving both active duty and reserve, with the rank of captain. He graduated from the University of Texas Health Science Center at Houston in 2001 with a Master's of Science in Nursing and is an Emergency Nurse Practitioner. He currently resides in Tomball and enjoys spending time with his wife and three children.
Tamika Lindsey, APRN, FNP-C a native Houstonian, earned her Bachelors of Science Degree in Nursing from Texas Womans University in 2003 and returned to TWU and completed her Masters of Science Degree in Nursing, as a Family Nurse Practitioner in 2010. Prior to becoming a Family Nurse Practitioner she worked as a Registered Nurse in the Intensive Care Unit and Emergency Room. Since becoming a Nurse Practitioner she has worked in both Internal and Family Medicine before moving to Emergency Medicine. Tamika enjoys traveling, reading, shopping, and spending time with her friends and family.
Chaim Sigler MPAS,PA-C received his undergraduate degree in Nutrition and dietetics from Wayne State University in Detroit, Michigan. Following his undergraduate degree, he moved to Las Vegas, Nevada and completed his Masters of Physician Assistant studies degree from Touro University College of Osteopathic Medicine. PA Sigler has worked in urgent care medicine as well as family medicine since 2009 and uses his background in nutrition to treat each patient as a whole.
When seeing patients in clinic, Chaim always goes the extra mile to make sure his patients are treated like family.
In his spare time, Chaim enjoys spending quality time with his wife and 2 sons outdoors, cooking and playing piano and guitar.
Stephen Mack DC, FNP, originally from Los Angeles, California has been a primary care provider since 2004. Stephen is alum of Eastern Michigan University where he majored in Biology in addition to being a scholarship football player. He graduated from Southern California University of Health Sciences with his Doctorate degree in Chiropractic Medicine. He recently earned his Masters of Science in nursing degree as a Family Nurse Practitioner from Prairie View A&M University located in the Houston Medical Center. Stephen is an avid sports fan and is passionate about health and fitness.
John McDivitt, MSN, RN, FNP-BC earned his Bachelor of Arts Degree in 1992 from The University of Dallas as well as his Bachelor of Science in Nursing in 1997 from Texas Women's University. In 2001, he earned his Master of Science in Nursing from The University of Texas Houston Health Science Center where he became board certified as a Family Nurse Practitioner. He has extensive experience in Emergency and Family Medicine. John is married and has one child. In his spare time he enjoys surfing, volunteering in activities for children with disabilities and spending time with his family.
Lenee’ Gordon RN, MSN, FNP-C has been a nurse practitioner since January 2016. She is a Houston native. She obtained her Bachelors of Science in Nursing through University of Texas at Arlington while venturing into travel nursing in California. She has been an Emergency Room RN for 9 years, including a leadership role at Memorial Hermann Texas Medical Center’s Level 1 Trauma Emergency Center. She then obtained her Masters in Family Nurse Practitioner in 2015 through the University of South Alabama.
She is married and has two French Bulldogs. She and her husband enjoy extensive travel, hiking, running, and cooking.
Virginia graduated from her Bachelor of Science in Nursing in 1996. She got her MSN in Family Nurse Practitioner from Fairleigh Dickinson University in New Jersey with honors.
She is a member of Sigma Theta Tau Honor Society, Texas Nurse Practitioners and American Academy of Nurse Practitioners.
She moved from New Jersey to Houston due to her husband's job. She has worked in the Emergency Department, urgent care and Family Practice. Virginia enjoys traveling, camping and spending time with family and friends. She is a mother of three with 2 dogs.
Michael Morris, PA is a Navy veteran who retired in 1995. Michael has been providing medical care for the last 35 plus years. He has provided medical care in many spots on the globe including Antarctica. Medically well rounded, he has working in orthopedics, urology, and the emergency room. Michael is a graduate of University of North Dakota and George Washington University. He is married, lives in Richmond Texas , and his hobbies include restoring old cars, travel, and photography.
Ammie Naples, FNP is a dedicated medical professional whose experience in the healthcare industry has spanned almost two decades and three continents. She has served in various capacities and specialties including but not limited to Ophthalmology, Occupational Health Care and Urgent Care. She obtained her Master’s degree in Family Nursing Practice at Prairie View A and M University, receiving highest honors as a Sigma Theta Tau awardee. She maintains a leadership role in the safety and security committee in Estates of Silver Ridge, Sienna Plantation where she and her family resides. She is a hardworking individual whose life’s goals are ruled with ethics and respect and geared towards the service of the community.
Monica Nolan-Salazar RN, MSN, FNP-C has been a nurse practitioner since 1998. She is a native Texan and was born in San Antonio and attended the University of Incarnate Word where she obtained her Bachelors of Science in Nursing. She then moved to Houston in 1994. She graduated from the University of Texas Medical Branch at Galveston with her Masters in Family Nurse Practitioner in 1998. Monica has extensive emergency medicine experience and family practice experience. Monica was recognized for her Excellence in Caring, and was given the award for provider of the year in 2010. She is married and has three children and one grandchild. She enjoys traveling with her family, cooking and is a Girl Scout troop leader. Monica is also fluent in Spanish.
Ivan Ranada MS, MPAS, PA-C received his undergraduate and graduate degree from Texas A&M University- College Station. After completing his internship at the Health and Wellness Institute in Providence, Rhode Island, he pursued his Masters of Physician Assistant Studies degree at Touro University College of Osteopathic Medicine in Las Vegas, Nevada. PA Ranada started practicing in urgent care in Bellingham, Washington. He and his wife, who is also a physician assistant, recently relocated to Houston to be closer to family and enjoy what Texas has to offer.
His research interests include health disparities and corporate health. He enjoys going on medical mission trips, cooking and traveling. PA Ranada grew up in Spain and is fluent in Spanish.
Tristan Richards is a graduate of University of Texas - Pan American and is a certified Physician Assistant who treats his patients like family. He chose to go into medicine because he genuinely wants to help his patients, and guide them in making educated medical decisions. Tristan has 4 years experience in general practice with emphasis in managing HIV in a family practice setting, which has given him the ability to manage each patient as a whole. His experience also includes involvement in 20+ clinical research trials investigating new HIV treatments. Tristan strives to practice innovative and comprehensive medicine to better serve the community. He recently relocated back to Texas with his wife and son in order to be closer to family. In his spare time he enjoys the outdoors and spending time with friends and family.
